🤝 Partner With Us: Brave Bags for Teen Transplant Patients

By joining the Brave Bag partnership, your hospital or center will receive:

  • Brave Bags delivered directly to your team

  • Printed materials and digital tools to connect patients to Transplant Teenz

  • Ongoing support and quarterly check-ins for restocking

We simply ask that you:

  • Designate a staff contact for communication

  • Distribute bags to eligible teens

  • Let us know when supplies are low

📝 How to Get Started

Please review the Memorandum of Understanding (MoU) below, then fill out the short form to confirm your participation. Once submitted, we’ll reach out to coordinate your first delivery.

MOU Inpatient Brave Bags

This Memorandum of Understanding (MoU) establishes a collaborative relationship between Transplant

Teenz, a nonprofit initiative dedicated to supporting teen transplant recipients, and your transplant center, hereafter referred to as the "Partner Site," for the purpose of distributing Brave Bags to eligible patients.

1. Purpose

The purpose of this MoU is to outline the roles, responsibilities, and mutual agreements between Transplant Teenz

and the Partner Site in the implementation and distribution of the Brave Bag Program, a supportive initiative

designed to uplift, connect, and provide resources to teen transplant recipients.

2. Scope of Collaboration

TRANSPLANT TEENZ AGREES TO:

  • Provide Brave Bags at no cost to the Partner Site for distribution to adolescent transplant recipients (typically ages 12–19 or grades 7th-12th).

  • Ensure each Brave Bag contains age-appropriate, safe, and culturally sensitive items.

  • Include printed information or QR codes connecting recipients to the Transplant Teenz community.

  • Initiate a quarterly check-in with the Partner Site to assess inventory, collect feedback, and coordinaterestocking as needed.

THE PARTNER SITE AGREES TO:

  • Distribute Brave Bags to eligible inpatient teen pre or post transplant recipients.

  • Store bags in a clean, secure area until distribution.

  • Designate a point of contact for communication and coordination.

  • Notify Transplant Teenz if supplies run low prior or during quarterly check-ins.

  • Share feedback and impact stories when appropriate and with patient consent.

3. Branding and Representation

  • Transplant Teenz will provide approved logos and materials for any co-branding or display.

  • The Partner Site agrees not to alter or reproduce Brave Bag contents or materials without prior written permission.

  • Both parties agree to use one another’s logos or photos publicly (e.g., on social media) only with mutual consent.

 

4. Liability and Safety

  • Transplant Teenz will make every effort to ensure that all items included in Brave Bags are new, safe, andappropriate for teens. However, the Partner Site assumes responsibility for final review and distribution of itemsto patients, and is encouraged to remove any item they deem unsuitable.

  • Transplant Teenz will not be held liable for any allergic reactions, injuries, or outcomes resulting from use ofitems once distributed.

5. Duration and Renewal

  • This MoU becomes effective upon the date of the final signature and remains in effect for one (1) year.

  • Either party may terminate the agreement with 30 days’ written notice.

  • This MoU may be renewed or amended upon mutual written agreement
